Rules, Terms and General Conditions
Anniversary Fee / Yearly Registration Fee
All Students will be charged an anniversary fee also known as Yearly Registration Fee. This fee is good for one year or until you drop out of the program. $75 per student / $130 per family.
Enrollment and Fees:
Class Registration: All classes require pre-registration. Enrollment is on a first-come, first-served basis.
Payment Due Date: All fees for programs and services provided by The JAM UAC are due on the 1st day of the month or upon registration.
Withdrawal: Parents must notify The Jam UAC to drop a student from class. A 15 days written notice via email to our Front Desk (admin@themjamurbanclub.com) is required to process your request.
Attendance: Regular attendance is crucial for skill development. Please notify us if your child will be absent.
Make-up Classes: Make-up days can be scheduled within the active session with our front Desk agent, by phone or in person. Note that enrollment for make-up days are based on a first-come, first-served basis and are subject to availability. Make-up classes are limited to 1 per session (4 weeks membership) or UNLIMITED for 3 month plan.
REMINDER: You are responsible for payment for your student's classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS until the time you notify the staff VIA WRITTEN NOTICE. Please do not rely on your student to verbally let us know that he/she will no longer be attending classes. If a student stops coming to class without notification then that student's account will be charged for the additional 30 days. This charge will be for holding the student's place in that class instead of offering that place to one of the many on a waiting list.
Class Attire:
Gymnastics/Tumbling/Ninjastics: Participants must wear comfortable athletic attire that allows for free movement. No jewelry (stud earrings are acceptable). Hair must be secured away from the face. Bring your own chalk.
Parkour: Comfortable athletic wear. No jewelry. INDOOR athletic shoes ONLY (clean soles)
Weight Lifting: Athletic attire and closed-toe athletic shoes.
Food and Drink: No gum or candy is allowed in the gym. Refillable Water bottles are recommended. Please Name Tag your personal items.
Parental Involvement: We understand your enthusiasm in supporting your child. However, to ensure the best learning environment, we ask that you refrain from coaching your child during class. Our qualified instructors are trained to provide proper guidance and feedback. Please allow them to do their job and avoid interfering with the class.
Facility Rules:
Arrival and Departure: Please arrive no more than 10 minutes before class and ensure prompt pick-up after class.
Waiting Area: Parents and guardians are welcome to observe classes from the designated waiting area. Please keep noise levels to a minimum. Respect the material, books, games and equipment at your disposition.
Prohibited Items: No food or drinks (except water) are allowed in the gym area. you are welcome to use the Party room for snacks and drinks.
Cleanliness: Please help us maintain a clean and safe environment. Dispose of trash properly. Inform us if you notice anything that need to be addressed.
Safety and Conduct:
Instructor Guidance: Participants must follow the instructions of the instructors at all times.
Respectful Behavior: We expect all participants to treat each other, the instructors, and the facility with respect.
Discipline: Disruptive or disrespectful behavior may result in removal from class and/or suspension from the program.
Injuries: Report any injuries to an instructor immediately.
Emergency Contact: Ensure your emergency contact information is up-to-date in your child's registration profile.
Medical Emergencies
In the event of a medical emergency at The JAM UAC, the safety and well-being of our participants are of utmost importance. Our staff is trained to respond promptly and effectively to any medical situation that may arise during activities, programs, or services. The following guidelines outline our procedures:
Emergency Contact Information: Participants are required to provide up-to-date emergency contact information upon enrollment. This information will be kept confidential and only shared with authorized personnel in case of a medical emergency.
First Aid and CPR Certification: Our instructors and staff maintain current certifications in first aid and CPR. They are trained to provide immediate assistance in the event of injuries or medical emergencies until professional medical help arrives.
Emergency Response Plan: The JAM UAC has a comprehensive emergency response plan in place. This plan includes evacuation procedures, communication protocols, and coordination with local emergency services to ensure a swift and organized response to any medical situation.
Parent/Legal Guardian Notification: In the event of a medical emergency involving a participant, The JAM UAC will make every effort to contact the participant's parent or legal guardian immediately. Timely communication will include details of the situation and the actions taken.
Medical Consent: Participants are required to have a completed and signed medical consent form on file, allowing The JAM UAC staff to seek medical treatment in case of an emergency when the parent or legal guardian cannot be reached.
Documentation and Reporting: Any medical incident, no matter how minor, will be documented by our staff. A report will be provided to the participant's parent or legal guardian, detailing the nature of the incident, the actions taken, and any recommended follow-up care.
Participants and their legal guardians are encouraged to communicate any relevant medical information or concerns to The JAM UAC staff to ensure the safety and well-being of all participants. By enrolling in our programs, participants and their legal guardians acknowledge and agree to comply with the guidelines outlined in this Medical Emergencies Policy.
Photography and Video Recording during class:
Parents are welcome to record or take photos of their child during class. Please ensure your camera is focused on your child only and avoid capturing other students whenever possible. If recording other children cannot be avoided, you must first ask the other parent(s)/guardian(s) for consent before filming, out of respect for each child’s privacy and public image.
Communication:
Contact Information: Please keep your contact information (phone number and email address) up-to-date in your child's profile.
Announcements: Important announcements and updates will be communicated via email, notifications in the app and/or posted at the facility.

Class - Payment Policy
Payment Due Date:
All fees for programs and services provided by The JAM UAC are due on the 1st day of the month or upon registration.
Accepted Payment Methods: Payments can be made using the accepted payment methods specified by The JAM UAC, including but not limited to credit/debit cards, check or cash. American Express is not accepted.
Late Payment Fees: A late fee of 5% will be added for each day that payment is not received after the 5th day of the month. Late fees will accrue daily until the outstanding balance is settled.
Notification of Late Payment: Participants and their legal guardians will receive a notification if payment is not received by the due date. The notification will include information about the late fee and steps to remedy the overdue payment.
Suspension of Services: If payment is not received within 10 days of the due date, The JAM UAC reserves the right to suspend the participant's involvement in programs and services until the outstanding balance, including any accrued late fees, is settled.
Refund Policy: Refunds will not be provided for late payments or for services that were suspended due to non-payment. Participants are responsible for ensuring timely payments to maintain their enrollment in The JAM UAC programs.
Payment Disputes: Any disputes regarding payments should be communicated to The JAM UAC's billing department within 15 days of receiving the payment notification. Failure to address payment disputes within this timeframe may result in the forfeiture of the right to dispute the charges.
Camp - Payment Policy
1. Registration and Payment:
Full payment is due at the time of registration to secure your child's spot in the camp.
We accept Visa, Mastercard, checks & cash.
2. Cancellation and Refunds:
General Policy: Camp registration fees are non-refundable. In the event of a cancellation, a credit will be applied to your The Jam member account. This credit can be used towards future camps or other programs offered at The Jam UAC.
Cancellation 3 Weeks Prior: If you cancel your child's registration at least 3 weeks before the first day of camp, a partial refund of 50% will be issued to the original method of payment, minus processing fees. Processing fees are 2.5% of the original amount.
Late Cancellations: Cancellations made within 3 weeks of the camp start date are not eligible for a refund to the original payment method. A credit will be applied to your The Jam member account.
Campers are expected to follow all rules and show respect to staff and fellow participants. The Jam UAC maintains a zero tolerance policy for disrespectful or disruptive behavior. If a camper is dismissed early from camp due to misbehavior or violation of our code of conduct, no refunds or credits will be issued for any portion of the camp missed as a result of early dismissal.
3. Returned Checks:
A $50 fee will be charged for any returned checks.
4. Discounts:
Please note that discounts are not available for camp registrations.

7. Acknowledgement
I represent and warrant that if I am purchasing something or paying for a service from The Jam UAC or from other merchants through The Jam UAC that (i) any credit card or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
I hereby authorize (if online payment is made or autopay information is provided) The Jam UAC to charge my ACH draft, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from class(es).
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.

The fire alarm system is in place for emergency situations only. Any individual who intentionally activates the fire alarm without a genuine fire or smoke-related emergency will be subject to disciplinary action.
Consequences for Children:
First Offense: Verbal warning and mandatory discussion with parents/guardians about the seriousness of false alarms.
Second Offense: Temporary suspension from The Jam UAC for 30 days.
Third Offense: Permanent expulsion from The Jam UAC.
Parental Responsibility:
Parents/guardians are responsible for educating their children about the appropriate use of the fire alarm system. Children must understand that activating the alarm when there is no fire is dangerous and disruptive.
Financial Responsibility:
In the event of a child intentionally activating the fire alarm without a genuine emergency, parents/guardians will be held financially responsible for the following costs:
False Alarm Fee: $150 - $300 (This fee covers the cost of emergency services response, staff time, and business disruption.)
Additional Costs: Parents/guardians may also be responsible for any additional costs incurred due to the false alarm, such as damages to the fire alarm system, staff wages and fines imposed by the fire department.
Explanation of Costs:
Activating a false fire alarm triggers a response from the fire department. This response involves multiple firefighters, fire trucks, and equipment, all of which come at a cost to the taxpayer and/or The Jam UAC. Additionally, a false alarm disrupts classes, stresses children and staff, and requires staff time to evacuate and manage the situation. It also leads to the cancellation of classes for the rest of the day.
Communication:
This policy will be clearly communicated to all parents/guardians upon enrollment and will be readily available on The Jam UAC website and at the facility.

At The Jam, we're committed to providing a safe, fun, and respectful environment for all campers. To ensure everyone has a positive experience, we ask all participants to adhere to our three core rules:
1. Respect Yourself
This rule is all about your safety and well-being. To respect yourself, campers must:
Follow directions for your safety. Our coaches are trained to keep you safe; listening to them is paramount.
Stay with your assigned group. This helps us keep track of everyone and ensures you're always under supervision.
Do not try new things without a coach present. Some activities require specific training and supervision.
Do not use the trampoline, warped walls, ball pits, or Rig without coach authorization and direct supervision. These activities can be high-risk if not used properly.
2. Respect Others
Creating a positive community means respecting everyone around you. To respect others, campers must:
Respect other campers' personal space. This means absolutely no pushing, hitting, or hugging without consent.
Be aware of your surroundings. Pay attention to where you're going and what's happening around you to prevent accidents.
Respect your coaches at all times. We have a zero-tolerance policy for talk-back, bad behavior, or poor attitude directed towards our staff. Our coaches are here to help and guide you.
3. Respect the First Two Rules
This rule is simple: by respecting yourself and respecting others, you're contributing to a fantastic camp experience for everyone at The Jam.
Important Information for Parents
Parents must discuss these three rules with their children prior to them attending camp. It's crucial that campers understand and agree to abide by these guidelines for a safe and enjoyable experience for everyone. It's also important for campers to understand that The Jam is an active camp. They will be participating in three core activities: Gymnastics, Ninjastics, and Parkour. This means campers will be consistently engaged in physical activity throughout the day.
Behavior Management and Consequences
We believe in positive reinforcement and giving campers the opportunity to correct their behavior.
Initial Redirection: In the event of a behavioral issue, a coach will provide one redirection to help the camper understand and correct their actions.
Parent Contact: If there is no improvement after the initial redirection, parents will be contacted immediately to discuss the behavior.
Camper Pick-Up: Should coaches determine a camper's behavior is inappropriate and poses a risk to their own safety or the safety of others, parents may be asked to come and pick up their child from camp.
Please note: Camp fees are non-refundable in cases where a camper is sent home due to behavioral issues.

MASTER RELEASE AND WAIVER OF LIABILITY
I, the Parent/Guardian both in my personal capacity and on behalf of the minor child
listed on my account, hereby acknowledges that participating in The Jam Urban Athletic
Club (“The Jam UAC”) programs involves inherent risks. In consideration of The Jam UAC
allowing Participant to take part in any of its activities or events, I agree to the following terms on
behalf of myself, my child, and our heirs and representatives:
I. Assumption of Risk.
I understand that participation in The Jam UAC activities (including but not limited to classes,
open gym sessions, “Open JAMs,” birthday parties, Parents’ Night Out events, camps, and special
events) involves inherent risks and hazards. These risks include, but are not limited to, strenuous
physical activity, gymnastics, Ninjastics, parkour and obstacle course challenges, climbing,
jumping, use of equipment and apparatus, and other athletic or play activities. I acknowledge that
such activities can result in falls, collisions, bodily injuries (ranging from minor sprains to major
injuries like broken bones or head injuries), paralysis, illness, or even death, as well as property
damage. I voluntarily assume all risks associated with my own participation and/or my child’s
participation in these activities, whether such risks are known or unknown, inherent to the activity,
or arise from the ordinary negligence of others.
II. Release and Waiver of Liability.
To the fullest extent permitted by law, I hereby release, waive, and discharge The Jam UAC, its
owners, officers, directors, employees, instructors, agents, volunteers, sponsors, and affiliates (the
“Released Parties”), from any and all liabilities, claims, demands, or causes of action that may
arise from or relate to any injury, illness, damage, or loss occurring as a result of participation in
The Jam UAC activities or use of its facilities. This release applies regardless of whether such
injury or damage is caused by the ordinary negligence (including but not limited to the failure to
inspect equipment or supervise Participants in accordance with standard practice) of the Released
Parties or otherwise. However, I understand that this release is not intended to waive any rights or
claims I may have for losses resulting from the Released Parties’ gross negligence or
willful/wanton misconduct, or any other liabilities that cannot be waived under Georgia law. In
other words, I am assuming the risk of ordinary negligent acts or omissions by the Released Parties,
but I am not releasing the Released Parties from liability for acts of extreme negligence or
intentional harm as permitted by applicable law.
III. Express Waiver of Claims for Ordinary Negligence.
I understand and agree that by signing this Agreement, I am expressly waiving and releasing any
and all claims that I or Participant may have against The Jam UAC, its owners, officers, directors,
employees, instructors, agents, volunteers, and affiliates (collectively, the "Released Parties")
arising out of the ordinary negligence of the Released Parties.
This includes, but is not limited to, claims based on negligent supervision, negligent instruction,
negligent maintenance of equipment or premises, failure to warn, or any other acts or omissions
constituting ordinary negligence in connection with activities at or sponsored by The Jam UAC.
This waiver does not apply to claims arising from gross negligence, willful or wanton misconduct,
or intentional wrongdoing by the Released Parties, to the extent such claims cannot be waived
under applicable law.
I acknowledge that I have had sufficient time to carefully read this Agreement and, to the extent
desired, consult with an attorney. I further acknowledge that I understand this waiver and release
is intended to be as broad and inclusive as permitted by law.
IV. Indemnification.
I agree to indemnify, defend, and hold harmless the Released Parties from any and all losses,
liabilities, damages, costs, or expenses (including court costs and attorneys’ fees) arising out of or
related to my or my child’s participation in The Jam UAC activities. This means that if any claim
or lawsuit is brought on behalf of Participant (or by me, my family, or any third party) for an
incident arising from our participation, I will reimburse the Released Parties for any attorneys’
fees, costs, damages, or judgments incurred by them. This indemnification obligation applies even
if the claim arises from the ordinary negligence of the Released Parties or others, to the fullest
extent allowed by law.
V. Emergency Medical Authorization.
Prior to Participant’s initial involvement in any The Jam UAC activity, I understand and
acknowledge that I must fully and accurately complete the Participant Information and Emergency
Contact Form on behalf of every Participant. I also understand that The Jam UAC does not provide
medical insurance for Participants and that I am responsible for any medical expenses incurred. In
the event of an injury or medical emergency involving Participant during any The Jam UAC
activity, I authorize The Jam UAC staff and representatives to administer immediate first aid and,
if deemed necessary, to obtain medical treatment for Participant (for example, call emergency
medical responders or transport to a hospital). I understand that The Jam UAC will make
reasonable efforts to contact me (or the emergency contact provided) as soon as possible in such
an event. I assume responsibility for all costs of medical care and transportation incurred on
Participant’s behalf. I agree to hold the Released Parties harmless for any such medical decisions
or emergency care rendered in good faith.
VI. COVID-19 and Communicable Disease Waiver.
I acknowledge the contagious nature of COVID-19, as well as other communicable diseases (such
as influenza, RSV, chickenpox, etc.), and voluntarily assume the risk that Participant or I may be
exposed to or infected by such illness by attending The Jam UAC activities. I understand that such
exposure or infection may result from the actions, omissions, or presence of myself and others,
including other participants, parents, or The Jam UAC staff. I agree that the Participant and I are
assuming all risks of illness associated with participation. The Participant and I will comply with
all applicable health and safety rules of The Jam UAC (including any guidelines for staying home
when sick or hygiene protocols). On behalf of myself and Participant, I hereby release and hold
harmless The Jam UAC and the Released Parties from any and all claims arising out of or relating
to any exposure to or infection by any communicable disease (including COVID-19) in connection
with our participation. This includes any claims based on the actions, omissions, or negligence of
the Released Parties or others, to the fullest extent permitted by law. I understand that this means
I will not hold The Jam UAC liable if Participant or anyone in our family becomes sick as a result
of participation, except in cases of gross negligence or intentional wrongdoing by The Jam UAC.
VII. Photography and Videography Consent.
I grant to The Jam UAC and its authorized representatives the irrevocable and unrestricted right to
take and use photographs and video recordings of Participant during The Jam UAC activities or
on its premises. I understand that these images or videos may be used for promotional or
educational materials, advertisements, social media, website content, or other marketing purposes
for The Jam UAC. Such use may be in print, digital, or other media, and no compensation will be
provided to me or Participant for such use. I waive any right to inspect or approve the finished
photographs/videos or printed/electronic matter that may be used in conjunction with them. If for
any reason I do not consent to this photography/videography release, I understand it is my
responsibility to notify The Jam UAC in writing and then fill out, sign, and return via email the
opt out form provided to me by The Jam UAC.
VIII. General Rules and Safety Acknowledgment.
I acknowledge that The Jam UAC has established rules, policies, and codes of conduct for the
safety and enjoyment of all participants. By signing this Agreement, I agree that I and Participant
will abide by the following rules and standards at all times:
(a) Obey All Instructions. Participant will follow the directions and safety
instructions of The Jam UAC instructors, coaches, and staff at all times. Participant will use the
equipment only in the manner directed and will observe all posted signs or verbal rules given for
each activity or area.
(b) Respectful Behavior. Participant will treat fellow students, participants,
instructors, and The Jam UAC staff with respect and courtesy. Participant understands that
bullying, horseplay, or disruptive behavior is strictly prohibited and that inappropriate language,
gestures, or behavior (including name-calling, teasing, disrespect, or rude remarks) will not be
tolerated. Any physical aggression (pushing, hitting, kicking, inappropriate touching, or otherwise
harming another child) will result in immediate removal from the activity. The Jam UAC reserves
the right to remove or suspend any participant for unsafe, disruptive, or disrespectful conduct
without refund. I understand that The Jam UAC staff and volunteers are mandatory reporters under
Georgia law and are legally required to report any suspected child abuse or neglect to the
appropriate authorities. This includes, but is not limited to, signs of physical abuse, sexual abuse,
neglect, or other forms of maltreatment. I acknowledge that any concerns observed or disclosed
during my child’s participation may be reported in accordance with O.C.G.A. § 19-7-5, and that
such reports are made in good faith as required by law.
(c) Facility and Equipment Use. Participant agrees to respect The Jam UAC facility
and equipment. Participant will not intentionally damage equipment or use equipment in
unauthorized ways. If I observe any hazardous condition or damaged equipment, I will promptly
report it to staff. I acknowledge and agree that I will be financially responsible for any damage,
repairs, or replacement costs necessitated by the actions of Participant beyond ordinary wear and
tear.
(d) Health and Fitness. I certify that Participant is in good health and physical
condition to participate in the activities. I will inform The Jam UAC staff of any chronic conditions,
allergies, disabilities, or special needs that Participant has, prior to participation. Participant will
not attend or participate in any activity if Participant is ill, has any communicable condition, or has
been advised by a physician not to engage in strenuous physical activities.
(e) Emergency Contact Information. I will ensure that current emergency contact
information is on file with The Jam UAC. If any of our contact information changes, I will update
The Jam UAC promptly. In any supervised drop-off event (such as Parents’ Night Out), I will
remain reachable at the phone number(s) provided and will promptly pick up Participant at the
designated end time.
(f) Supervision of Minors. I understand that The Jam UAC’s staff will provide
general supervision during structured classes or events, but parents/guardians are ultimately
responsible for the behavior and safety of their minor children. I will ensure that any siblings or
children not actively participating in a class/event remain supervised and out of restricted areas. I
also agree to any specific supervision requirements set by The Jam UAC for certain age groups or
activities (see below for provisions regarding very young children). I understand that children not
attending classes are not allowed to run, practice drills, or play games in the common areas such
as the lobby, bathrooms, or lunchroom (also referred to as the party room) but rather must remain
seated and quiet in the front waiting area under a parent’s supervision at all times. I further
understand and acknowledge that I will be financially responsible for any costs or expenses
incurred due to the actions of any sibling or child not actively participating in a class or event
under my supervision, such as the cost of a Fire Department visit if said child pulls a fire alarm.
(g) Reporting Injuries or Issues. I will immediately report any injury, accident, or
unsafe incident involving Participant (or that Participant witnesses) to a The Jam UAC staff
member. I will also report any unsafe situations, rule violations, or anything that poses a risk of
harm in the facility.
IX. Special Provisions for Children Under 3 Years Old.
The following additional rules apply if Participant or any child I bring to The Jam UAC is under
three (3) years of age (toddler age):
(a) Restricted Activities. For safety reasons, children under age 3 are not permitted to
participate in The Jam UAC’s standard gymnastics, ninjastics, parkour, or open gym activities
alongside older children. Very young children may only engage in activities that are specifically
designated as age-appropriate (for example, playing in a toddler-only area or class specifically for
toddlers).
(b) Parental Supervision. A parent or responsible adult caregiver must remain with
and directly supervise any child under age 3 at all times while on The Jam UAC premises. This
includes keeping the child within arm’s reach in any play area and ensuring the child does not
wander into areas or onto equipment not intended for their age. I understand that The Jam UAC
staff are not responsible for individually supervising children under 3 in open gym or mixed-age
settings. I accept full responsibility for the safety of any toddler under my care while at The Jam
UAC.
By adhering to the above provisions, I aim to help maintain a safe environment for my child and
others. I understand that if I fail to abide by these rules or supervision requirements, The Jam UAC
has the right to immediately remove Participant and/or the toddler from the activity or facility.
X. Waiver of Jury Trial.
To the extent allowed by law, I (on behalf of myself and Participant) voluntarily waive any right
to a trial by jury in any lawsuit, action, or proceeding involving the Released Parties that arises out
of or relates to this Agreement or participation in The Jam UAC activities. This means that in the
unlikely event of a legal dispute, any such dispute shall be decided by a judge without a jury. I
understand that I am giving up the right to have a jury of my peers decide any claims and that this
waiver is made knowingly and voluntarily.
XI. Venue and Choice of Law.
I agree that this Agreement and any dispute or claim arising from participation in The Jam UAC
activities shall be governed by the laws of the state of Georgia, without regard to its conflict of
laws principles. Any lawsuit or claim that may be filed against the Released Parties shall be
brought exclusively in the state or federal courts located in Chatham County, Georgia. I consent
to the jurisdiction of these courts and agree that no other courts shall have jurisdiction or venue
over such claims or lawsuits involving the Released Parties. In summary, Chatham County,
Georgia is the designated venue for resolving any legal matters that may arise in connection with
this Agreement. In the event The Jam UAC is the prevailing party in any dispute arising under this
Agreement or the activities contemplated hereunder, The Jam UAC shall be entitled to recover
reasonable attorneys’ fees and costs.
XII. Acknowledgment and Understanding.
I certify that I am at least 18 years old and that I am the legal parent or guardian of the minor
Participant named above. I have read this entire Master Release and Waiver of Liability (or had it
read to me) and I fully understand its contents. I understand that by signing this Agreement, I am
giving up substantial legal rights on behalf of myself and/or the minor Participant, including the
right to sue the Released Parties for certain claims. I acknowledge that I have had the opportunity
to ask questions and consult with an attorney if I so chose, and I am signing this document freely
and voluntarily without any inducement or pressure.
I intend for this Agreement to be a complete and unconditional release of all liability to the greatest
extent allowed by law. If any part of this Agreement is found to be invalid or unenforceable, I
agree that the remaining portions shall remain in full force and effect and will still be binding on
me. This document constitutes the entire agreement regarding the matters herein, and it supersedes
any previous agreements or understandings related to these matters. This Agreement shall be
binding upon and inure to the benefit of the parties hereto and their respective heirs, legal
representatives, successors, and assigns.
I agree that a photocopy or electronic version of this executed Agreement shall be valid and
enforceable just like the original. Electronic signatures (or electronic acceptance through an online
system) shall be deemed equivalent to an original signature.
By signing below, I acknowledge that I have carefully read and understand this document. I affirm
that I voluntarily agree to all the terms and conditions stated above, and I understand that these
terms are binding upon me, Participant, and our respective family, heirs, estates, and legal
representatives.
